Matriculation Requirements

Applicants who receive letters of acceptance to the Master Biomedical Sciences (MBS) Program will have two weeks to either accept or decline their place in the MBS Class of 2020. Students accepting their place must submit an Acknowledgment of Acceptance and Confirmation form and a $200 non-refundable matriculation/seat deposit

Matriculation Requirements

If not already completed, submit official transcripts from all prior colleges and/or universities. An official transcript including the bachelor's degree conferred date is required. Official transcripts can be mailed to the UIW Graduate Admissions Office or sent electronically to admis@uiwtx.edu.

  • August 1, 2019 - Complete Castle Branch requirements

Castle Branch is a national background screening and compliance management tracking system that will be utilized by UIWSOM for maintenance of all required documentation for all our UIWSOM MBS students. The package includes a Criminal Background Check, Drug Testing, and Document Tracking of all required immunizations and forms. The UIWSOM MBS Package is UK72. 

Students are advised to view the Student Tutorial before beginning the process to Castle Branch at myCB Student Tutorial.

Complete the following tasks in Castle Branch: 

  • Background Check and Drug Screening

    The UIWSOM Master of Biomedical Sciences requires a criminal background check and drug screen for all entering students.  The background check and drug screening are performed by Castle Branch*. The background check includes criminal history and previous address information. 

    The Castle Branch system will conduct the required criminal background check, drug testing and management of all immunization documents. The package price is $107 and is not included in tuition and fees. Please log on to begin the process.

    The deadline to complete this required task for the UIWSOM MBS class of 2020 is August 1, 2019.
  • Immunizations and Physical Exam
     

    According to the Department of State Health Services, the Texas Administrative Code Chapter 97.64, the following immunizations are required for health professionals:

    Following acceptance to the program, all students are responsible for providing official proof of up to date immunizations to include the following and any other childhood immunizations and/or titers:

    • Meningitis Vaccination - All students under the age of 22 are required to have a record of the Meningitis vaccination on file
    • Hepatitis B
    • Tdap or Td booster recorded in the past 10 years.
    • MMR (2 doses)
    • TB skin test or negative chest radiograph (must be done annually)
    • Varicella Zoster vaccination, titer or history of (chicken pox) disease; booster will be required if immunity is not achieved; official documentation must be provided in any case.
    • Flu vaccine (annually)
    • Physical exam

    UIWSOM will hold registration of any student who does not provide proof of required immunizations by August 1, 2019.
  • Proof of Medical Insurance

    All full-time students are required to have medical health insurance coverage either through the UIW or through another individual or family plan. Students are required to demonstrate proof of coverage with major medical insurance at levels required by the University.  A student may choose their own personal insurance plan as long as it is comparable in coverage to that offered by the University.

    To decline the Student Health Insurance, visit: Waivers. The form will be available beginning June 1, 2019. Additionally, upload a copy of your insurance card to Castle Branch. (The deadline and additional details can be found on the Health Services website.)

    **All full-time MBS students will be automatically billed for the UIW Student Health Insurance once you are registered and the tuition/fee bill is generated. If you have personal health insurance, please submit the waiver form by the date listed on the Health Services website to avoid the extra charges.

  • August 1, 2019 - Complete prerequisite coursework

Official transcripts indicating success completion of prerequisite coursework are required. If the deadline cannot be reached, contact the Office of Admissions immediately.

  • August 14-16, 2019 - Attend the mandatory UIW MBS orientation week
