- Prior to submitting request, complete Hazardous Waste for Disposal Tag provided by EHSRM. If you do not have one, please indicate this in the request form below or download Hazardous Waste for Disposal Tag (PDF). Downloading label gives you the option to complete label and print rather than wait for delivery of label. Once completed, continue following the steps below.
- Affix Hazardous Waste for Disposal Tag to appropriate container requiring disposal.
