You may download these forms to conduct business with the Registrar's Office. After completed, send the form to the appropriate approval entity (e.g., your dean, school/college curriculum committee, etc.) prior to routing to the Registrar's Office. You may select between documents present using Microsoft Word or through Adobe Acrobat (PDF) if you do not have Microsoft Word. To use the PDF forms, you will need to install the Adobe Acrobat program on your computer, if you do not already have Adobe Acrobat.
