Beginning Fall 2007 the University will accept Student Health Insurance waivers for Domestic students. Only domestic students are allowed to waive insurance, International Students are not allowed to waive coverage per University of the Incarnate Word policy. Waiverswill only be accepted online to STUDENT ASSURANCE SERVICES, INC. on or before the stated waiver deadline published in the respective course schedule ( See Academic Calendar).
Health Insurance Waiver
The University of Incarnate Word will no longer accept insurance waivers in the Business Office. Make sure to have all your personal health insurance information with you when ready to submit the waiver.
Here are the steps to get to the Student Health Insurance Waiver screen:
- Log onto (www.sas-mn.com),
- Select College, Students:, click on “Look up school”;
- At the pull-down menu, select “Texas”.
- At the pull-down menu, select “University of the Incarnate Word – Domestic Students”;
- Select Waiver Form Online
After you have completed the waiver and clicked the “APPLY for waiver” a screen with a waiver receipt form will appear. Print this screen for your records. An email will be sent to the email address provided, confirming that you have successfully completed and submitted your online waiver information. If you do not receive a confirmation email, please contact Student Assurance Services, Inc. at info@sas-mn.com.
Credits are issued 10 days from last day for 100% refund of enrolled term(s). We recommend that you review your student bill periodically to insure that credit has been issued to your account.
Parking and Bookstore Waivers
UIW will not be held responsible for lost mail. All forms downloaded from this page must be submitted to the corresponding department with original signatures. No faxed or reproduced copies will be accepted.
Completed waivers must be received BEFORE THE STATED WAIVER DEADLINE PUBLISHED IN THE RESPECTIVE COURSE SCHEDULE FOR THE ACADEMIC YEAR 2008-2009. Parking waivers must be submitted at the beginning of each semester.
