- ___ Please apply to the University officially, before you turn in the 1905 to UIW VA Coordinator
- ___ Please contact your vocational rehabilitation counselor and obtain your 1905 for the University of the Incarnate Word. All 1905s must be received by, UIW VA Coordinator, (210) 829-2705 or (210) 829-2756
- ___ Turn in a copy of your 1905 when picking up your supply voucher at the Business Office only if you are going to go over the maximum amount of $75.00.
- ___Supply and book vouchers are only available from the UIW Business Office. You will not be able to obtain one through the UIW Bookstore.
