- ___ Please apply to the University officially, before you turn in the 1905 to Brandy Valle.
- ___ Please contact your vocational rehabilitation counselor and obtain your 1905 for the University of the Incarnate Word. All 1905s must be received by Brandy Valle, Registrar’s Office. (Brandy Valle (210) 829-5850).
- ___ Turn in a copy of your 1905 when picking up your supply voucher at the Business Office only if you are going to go over the maximum amount of $75.00.
- ___Supply and book vouchers are only available from the UIW Business Office. You will not be able to obtain one through the UIW Bookstore.
