1. Goto the ATEP Clinical Instructor Resource Page and open the evaluation tool for the level student in which you are currently working. (Level 1 Midterm , Level 4 Final, etc.)

2. When the PDF Password Protected Document is opened you will be prompted for a password. Note: You can access your password from the CIE or Director of Athletic Training Education

3. Fill in the PDF Forms by typing in the information and using the check boxes. This process takes approximately 5 minutes.

4. Print the form.

5. Have the student read over the evaluation form, discuss the evaluation and each of you sign off on the form.

Note: It is the student’s responsibility to get the evaluation form back to the CIE within the requested date. Note: In the event that it is not received, the CIE will contact both the student and clinical instructor.
